Photoshop: Pen tool cross hair is several pixels off (windows - Surface Pro 4)

Using a surface pro 4 and surface pen 

When ever I make a line the pen crosshair will always be several pixels off of where my pen actually is.
I don't believe it's the pen or the surface because in other digital art apps the pen everything is fine.
This isn't a massive problem but it's very annoying when I'm doing little details

What product/version are using?

I assume you're on Window 10. Is Windows 10 up to date?

Hi Soy,

I'm looking at the precise brush crosshair while I am painting with a 1px brush.  I do see a small offset which I would say is only off in the side to side direction.  Up and down, the cursor appears to be well-aligned with the tip of the stylus.  But it does look to me that the cursor is a few pixels to the right of the stylus tip.  The resulting strokes appear to be coincident with the cursor, at least.  Is this what you are seeing, or is it worse?  I also see similar offset in MS Paint.  
To me, the problem is worse when the stylus is at an angle to the screen.  When I hold the stylus (unnaturally) straight up and down (perpendicular to the screen surface) I'm not sure I can see the problem.  What does it look like to you?

I switched pen tips and the problem is a lot smaller with it only moving 2-3 pixels to the left 
Maybe my old pen tip was broken idk
