Often when you place a smart object into Photoshop and transform it, the transform is re-set and you have to transform it again.

What version of Photoshop?
And could you provide a bit more information? I haven't heard of that happening before, unless someone intentionally cancelled the transform.

Currently on Photoshop 14.2. Pretty sure it was an issue from 14 though.
I'm on OS X 10.9. All standard mac hardware.
I convert anything to a smart object. CMD+T for a transform, hit 'enter' bottom right of my keyboard below the numerical keys (or double click) to confirm and the transform immediately re-sets. I then try and transform again and it works fine.

I haven't heard of anyone else having that problem.
Any chance you have the enter key setup to do something in an OS shortcut?

I have suddenly started having this problem as well. I think it started a while ago but I just put it down to me moving too fast.
Today, tho, I can't do any transform with a smart object. I tested it on another layer. No problem. But if my layer is a smart layer or if my selection of multiple layers has a smart layer in the set, I cannot type in percentages or grab corners and drag.
And, here is the first twist: It only happens if that smart layer has a mask on it! Masks within the smart object do not cause this behavior. Only if there is a mask attached.
However - twist 2: If I unlink the layer mask, I can transform either the mask or the smart object. If I relink the mask, then I can transform both together like it always should.
I went to anther smart object in the same psd (I like smart objects!) and it would not let me transform.
Unlink the mask, and then relink. Same problem.
Unlink, transform the smart object, undo, relink. No problem.
So, there's a work around, at least on my system.
But, it is very odd behavior!
