In the latest CC 2015, Toolbar preferences aren't inheriting changes to keyboard shortcuts. When you make changes to the toolbar, program defaults are preserved in there, so your keyboard shortcuts are ignored, though they still persist in the keyboard shortcuts dialogue. The toolbar settings seem to have precedence, however.

I really like the idea of the toolbar customization, but it's a little clumsy atm. For instance, I don't think the average user is expecting that if they deselect the 'fullscreen mode' button, that it'll kill their ability to use the F key. It seems a bit like you're muddling functionality between this screen and the keyboard shortcuts screen. I had to change in both places to make stuff work. I get that you want an easier, quick place to customize common tools, but make the two functions talk to one another and it'll be worth doing.
