Lightroom: Thumbnails don't render with "preview unavailable for this file" error when importing from camera or other media

when i connect any piece of equipment from professional camera to iphone to my mac and try to download photos at least 50% - 75% of the thumbnails dont show and all i get is a piece of text where the thumbnail should be that says "preview unavailable for this file"
The file downloads OK but this is a phenomenon only experienced in LR4. LR3 didnt have this problem - Can you help?

I spent a hour with one of the Adobe help reps last week but got nowhere

Are you connecting these devices via the same cable, IO port?

no, the iphone connect via its cable and my camera via a mini usb cable. not a problem with LR3 only happening with 4

same thing still happens in LR6, all updates installed as of January 2016. Importing from iPHone 6s plus (that has thousands of images in internal memory) almost invariably results in many thumbnails not rendering at "import" screen, with notice "preview unavailable for this type of file". The files are all regular iPhone jpegs. When will we see the end of this bug in LR, I wonder???
