LIGHTROOM 5.3: Synchronize folders tries to reimport images

I am using LR5.3 and for some reason every time I "Synchronize a folder" it tells me all of the images are new and highlights the same number to remove.
It will show me all of the missing images and they are all there.
If I reimport anyway, instead of putting them in the subfolder, it adds them into a folder in the top of the folder list.
It makes no sense.

Just checking further - it is doing this for every image in all of my folders.
Choosing an image and selecting "Open in finder" opens the correct directory in finder, so the image is there. Then selecting "Open in Library" selects the correct folder in Lightroom, so it all seems correct.
If I allow it to delete the images and reimport them it creates a new folder at the top of the folder list.
Then if I select "Show Parent Folder" I get the attached delightful little error.
