Lightroom 6.4: Smart collections not updating on Windows 10

I've got a smart collection that looks for this:

Photo is not flagged
AND
Photo is exactly 1920x1080
OR
Photo is exactly 1280x720

Worked a charm when I added this smart collection. But now that I've added more picture to the library, they are not added to this smart collection, even though they meet these criteria perfectly. It doesn't seem to matter how these picture get to be added to the library. It seems like my smart collections are only ever populated once - when they were created.

So, two things must happen:
1) Fix Lightroom so that it updates smart collections properly
2) Add a trigger to update smart collections manually, just in case

And optionally:
3) Add a way to see why a picture is NOT in a smart collection, i.e. why it doesn't meet the criteria, or which criteria aren't met, if LR thinks so.

As of currently, smart collections cannot be trusted as I see it.

This is on Lightroom 6.4 on Windows 10.

So a smart collection also seems to update itself when I modify the criteria. Good to know. Still, it's a tedious and easy to forget workaround.

Well, it seems various pictures throughout my library that *clearly* match the criteria, still aren't included. Why is that? It has to be a bug. There is *no* way my criteria could be wrong, they are crystal clear. It looks for 1920x1080 pictures, and I've found many that are 1920x1080, and they are not included. Seriously, why??

Can you post a screenshot of the Smart Collection criteria window?

And/Or right-click on the Smart Collection you're having trouble with, choose "Export Smart Collection Settings...", save that file, post it to some shareable location, like Dropbox, and reply with a link to the file?

A link to one of the images that you feel match the criteria would be helpful too.

I can reproduce the problem with my LR CC 2015.5 / OS X 10.11.4, using the smart-collection definition below.   When I import a 1920 x 1080 JPEG, it does not appear in the smart collection.  However, if I exit and restart LR, the smart collection then shows the newly imported pic.

In general, LR does automatically refresh smart collections -- if it didn't, we would have seen hundreds of complaints, given how many people rely on them.  There is something about this smart collection in particular that is causing the bug.
