Photoshop: How do I include layers when saving as PDF?

Just recently, every time I save a PDF with layers, it automatically flattens the file. I made sure that they layers box is checked and I used just about every preset (except High Quality Print, which saves the layers).

When I save the file as a PSD, the layers are preserved. I know this may be a silly beginner's question, but I can't figure it out!

After the Save dialog, there is an options dialog for PDF -- and that dialog has more options that can affect saving of layers.

In the PDF dialog, under "General", you want to make sure that "Preserve Photoshop Editing Capabilities" is checked.

The box was grayed out unless the preset was High Quality Print.
