Photoshop: How do I remove dust without losing details?

On very big details from old paintings one can find lots of dust. On newer paintings there are lots of specular highlight points...

To remove it, the best I could find is to play with the parameters in Dust&Scratches after a selection made. But making all these selection is a very tidyous work because if you select to much, you loose other details like craquelure, which should stay.
In the picture attached you can see that with these settings, some portion of the fine dark lines are already gone...
Making finer selections is to much work on very large pictures. Playing with diff erent settings is not always good for the entire painting.
Who has a good tip for me ?

Not all details can be lost, but disturbing highlights have to go ...
