You are not getting any themes (empty second panel) which could be the cause of the problem.
What happens when you click on 'Photo Book'. Do you see any theme there in second panel? Does PhotoBook also crash on clicking 'Ok'?
If possible, send screenshot for PhotoBook as well.

Your Photo Collage dialog should look like:

The PhotoBook looked normal. Only Photo Collage doesn't have Theme. Do you know how can I fix it?

Your Photo Book also doesn't have themes...see second panel it's empty.
Give me sometime I'll get back to you on how to fix it.

Meanwhile, please do following 3 things:
1) Share the result whether your Photo Book crashes on clicking 'OK'?
2) Try different sizes from first panel. Let me know if you see any theme in the second panel while selecting sizes.
3) After you've done first 2 things, close Editor and install Adobe AIR from http://get.adobe.com/air/ . Make sure that you install Mac version of Adobe AIR.
After installing AIR, launch Editor again. If you start seeing themes in second panel, please let me know.

There is no Theme on any sizes on second panel. After selection, they all crashed too.
I tried all 3 steps. It still crashed (with new Adobe Air)
I just realize that there is no Theme on any of these: Photo Book, Greeting Card, Photo Calendar, Photo Collage and PhotoStamps.

The crash problem is because of missing themes.

1) Please re-install Photoshop Elements. Re-installing will install themes also.
2) After re-install, try Photo Collage, Photo Book etc workflow
and let us know the result.
3) Also, after re-install, please share the screenshot of following location:
'/Library/Application Support/Adobe/Photoshop Elements/9.0/Photo Creations'

I've tried re-install (w/o uninstall) and re-install by 1st uninstall. Both gave the same result, crashed unexpectedly. Also there's no theme in PSE but I can see theme folder with files inside at location: /Library/Application Support/Adobe/Photoshop Elements/9.0/Photo Creations

Yes, they do. All of them have files, that's visible. Is there any way to insert Theme into it?
