Photoshop 2020: Why are there generic icons for adjustment layers sometimes?

This is also in PS 2017 and probably all after (on PC Win 7).

Regularly, when adding adjustment layers or running an action, adjustment layers don't get their proper icons but will all show with the same icon like here ...

You have set the thumbnail size in the Layer Panel Options to 'None' instead of one of the three icon sizes. Click on the three lines in the upper right corner of the layers panel.

Could have been, but I have not *ever* done that. I use the smallest size.
It is different several times within the same session.

Some actions take 4 seconds and then leave you clueless with 30 generic icons... But randomly happens when adding adjustment layers manually too.

If no one else has this(? it's easy to not notice)), maybe it's some extension clashing.
I might try with extensions/plugins OFF.

Freshy started PS and I had it right away.
Also, there is a clear difference if you opt for no thumbnail. The spacing is tighter and the icons always select squarely, even if document is not.

Smallest thumbnail option:

I've had this "forever" but often didn't realize it. It's more obvious when an action leaves you with 30 generic icons...

In case someone thought I was some kind of exception, I just noticed the same thing watching an old tutorial with PS 2015.5, confirming it's a longstanding bug... (Curves layer has wrong icon)
