Photoshop "Save As" doesn't work correctly when passed a raw file from Lightroom

This is a bug report for Photoshop CS6 (13.0 x64 for Macintosh) and Lightroom 4 (4.1). I don't believe that this bug occurred for Photoshop CS 5 + Lightroom 4.

I have a custom file template for the Edit (cmd-E) command in Lightroom. This template is not actually what I want, but is the closest I can come to what I want in Lightroom, so I use it. See the attached image for the custom file template settings.

If I open a raw file in Photoshop from Lightroom using this template (cmd-E), edit the file in Photoshop and then do a Save As, I can save the file with a chosen name. The bug comes when I do a subsequent Save command in Photoshop. At this step (a Save following a Save As), Photoshop does not save the file with the name that I have given it in the previous Save As, but instead does a second Save As, and gives it the name suggested by the template. So now I have two files—one with the name given in the first Save As, and a second with the name suggested by the template.

There is some subtlety to this that I don't understand, because I cannot replicate this bug under all conditions, but if you get back to me, I can give you a complete recipe for replicating this bug.
