Photoshop CS6 save for web error 'The operation could not be completed'

I'm trying to save a GIF pixel animation for my deviantART icon. I have the animation done and saved as PSD, but when I try saving for web an error pops up saying 'The operation could not be completed'.
I'm using a Windows XP. I searched for answers and none helped, I also deleted the 'Adobe save for web prefs 12.0' and there was a '13.0' file as well but that didn't help. Also, I was wondering if the folder 'C:\Documents and Settings\(Username)\Application Data\Adobe\Plugins\Adobe Save For Web CS6\Photoshop' being empty has anything to do with it Also, like in this screen shot it makes Photoshop load funny
Can someone help me resolve this please?

That error dialog does not look like a Photoshop error dialog, nor is the message one that I'm familiar with in Photoshop (it should always have a reason).

The UI drawing funny could be a problem with your video card driver, or a utility or other application on your system causing problems.

Try to solve the UI problems first, so you know that something else isn't messing up Photoshop. Start by updating your video card driver from the GPU maker's website (not Microsoft).

The 12.0 prefs are for Photoshop CS5.
The 13.0 prefs are for Photoshop CS6.

No, I just updated my Graphics Card, I'm not sure what you mean by UI but I just tried to save my animation again and it came up with the same error message

UI == User Interface. You're seeing huge problems with the UI - so something is wrong with your system. You need to solve the system problems first.

I have a very similar problem on my MacBook Pro (13" Core i7 mid 2012). CS6 Creative Cloud subscription, all the latest updates. Mountain Lion 10.8.2 with latest updates (but the problem happened on 10.8.1. too). Can't save to any format and happens every time. Today I reinstalled Photoshop - still get the same error.

I know this post was three years ago, but in case the issue persists, check the image size (378 x 525 px) and compare it to the canvas size. The canvas size may be in inches. If this is the case, then lower the canvas size to match the image size and try saving to web again. This should solve the issue, if it still is a problem. I hope this helped!
