Photoshop CS6 access violation on startup

I have occasional crashes on startup of Photoshop. I just downloaded this version on 8/25/12. Usually this happens right after the main window appears. The crash dump shows the failure to be occuring in ntdll heap code.

With full page heap debugging enabled in Windows, it crashes every time, much earlier, before the splash screen appears, at Photoshop+0x103c8aa. I would guess that Photoshop is misusing some piece of memory in the heap, which may or may not cause a later crash if full page heap debugging is not enabled.

I am running on a VAIO Z (SVZ131190X) with switchable graphics. The problem started showing up when I installed the Power Media Dock and thereby started using the AMD Radeon graphics processor, so I thought this was related. But I disconnected the PMD and went back to the Intel HD 4000 processor, and saw the same behavior (crashes at the correspondingly same times with and without full page heap debugging, with the access violation at the same code byte). I think that by chance I didn't get any crashes in the few times I ran Photoshop under the Intel processor, before installing the Power Media Dock.

Version is Photoshop CS6 13.0 x64. Stack trace follows. I can provide full crash dump on request (it's about 400 MB).
