'Photo has Develop adjustments' icon shown on unedited photos

I have just realized that hundreds or even thousands of photos in ma catalog has 'Photo has Develop adjustments' icon shown even though they have never been edited.
There are some collections I have that I have never touched. There are collections that I have edited some of the photos but there are some other that I have not touched but the icon is show.
There are some collections that all photos are marked with the icon but there are some collections that only some of the photos has the icon and never been touched by me.
I don't see any consistency there.
I have checked some of these photos and I don't see any changes even though the icon is shown.
Not sure if this is something new after latest version of app installed or it was there long way before.

Hi Michal,

Did this change happen after the update?
Also, can you verify whether you're seeing this with any of the freshly imported images?

Make sure that none of the presets are applied at the time of import.

Hi Bhargav,
I didn't notice it earlier. But I can't say it has happened after update though.
I am sure I have no presets applied at the time of import. I have never setup any presets on import.
As mentioned before I have some old catalogs that I have not edited any of the photos and only some of them has these icons - not all of them. 
The strange thing is that when I check such a photo there are no changes made at all - I don't see any modifications.

Modifications are not always immediately apparent. It could be a different camera profile, for example. The first thing I would check is the History panel.
