Lightroom: Multiple imports to a named folder might create multiple folders with the same name

Lightroom 4.2 on Mac OS X 10.7.5
Have used LR4 on my PC extensively but I have not upgraded LR to 4.2 on the PC yet, i.e. I don't know if the problem is linked to the operating system or to LR4.2.

Problems with importing or moving pictures. If "import to a folder" is checked and then unchecked, the folder list is not updated and will contain ghost folders.
1. Two or more folders that should be imported and merged into one folder
2. First folder is moved to my external drive to a folder with a new name
3. Import the next folder and the check box "Import to folder" is checked and remembers the previous import's folder name. The Destination folder list now contains two folders with the same name.
4. Problems might appear if you try to rename or drag pictures from one folder to another. It looks ok at first, but upon completion of the move, an error message appears.
