Photoshop CS5: Marquee tool will not create perfect square of circle holding down shift

I need to make perfect squares and I usually select with the marquee holding down shift and then crop. The marquee tool has, however, been acting strangely for the past few days. It won't make a perfect square so when I crop and check the dimensions they are about 12 pixels off. This problem persists even when I reset the marquee tool.

I restarted the computer and reset the tool again and still no luck. I have seen this problem already after searching forums (including this one) - but I am not having the same one. The one I've seen pertained to Mac users having a jumpy marquee tool that was one pixel off. Mine is not jumpy, it stays where I leave it - just when I hold down shift it comes out about 12 pixels off. I am using Windows.

That sounds like you have an aspect ratio entered for the marquee, but resetting the tool should have cleared that.

Does your image have an aspect ratio? (check View->Pixel Aspect Ratio, which should be set to Square)
