My previews look like theyre being masked by a map icon, I have actually used this map icon in a file, so its familiar. I'm not sure why all the psd previews are being masked by it now though, has anybody had a problem like this? Is there a way to just reset the icon?

What program and version are you using?
What OS version?
What file format?

That looks a bit like a bug on MacOS where they don't read alpha channels correctly (despite many bug reports to them about the issue).

Sorry, should have included that:

- Photoshop CC 2014.0.0
- Mac OSX Yosemite
- PSD (this is the only one it happens to)

Looks like Apple fixed it, then re-broke it.
It's because you don't have transparency, but do have an alpha channel (saved selection or spot color). Apple's code is just reading the file incorrectly.
