Lightroom 4: "Don't Import Suspected Duplicates" FAILS for two identical files (same name, same MD5)

Using LR 4.4, I'm seeing duplicate files not detected when the files have the same name and identical content (same MD5).

I've isolated a small test case where I have a directory with a small number of images (taken at the same time with the same camera.)

I can copy this directory to a different location, then try to import it and one of the files will not be detected as a dupe.

I've tried re-reading the metadata for the existing library file.

Files have the same name and same checksums.

This is not an issue just for this one file, I have a 35k database that is flagging 9k dupes when I import from a backup.

This is probably not going to cause any action from Adobe unless it is happening in the current production version of LR which is 5.5, soon-to-be 5.6.

Do you have a way for someone else to replicate the problem with a newer version of LR?

I am having the same problem, and I am in 5.5. It does not recognize the photos as duplicates even when I try to import the SAME folder a second time, that the original pictures are read from. (I accidentally hit, "delete", not "remove fro disk" when deleting rejected photos, and was trying to re-import the photos, hoping that only the rejected photos would show up in the import)
