My guess- the plug-in is causing it, but without more details from you it remains difficult to suggest an answer.
What plug-in?
What type of file?
What options are set for the plug-in in the External Editing preferences?

Plug-in is Topaz Adjust 5, the file is "1801_Sedona'18_072-HDR-Edit.tif", External Editing preferences: File Format: TIFF, Color  Space: ProPhoto RGB, Bit Depth: 16 bits/ component, Resolution: 300, Compression: none

Note: when I edit in OnOne Raw, and re-import, no problem, I can see the image, which adds credence that the plug-in (Topaz Adjust 5) is causing the issue.

Does the ...-Edit.tif look ok in Photoshop?  In other words is something wrong with the file or is something wrong with LR?

I've seen this when re-importing from PortraitPro and sometimes from Photoshop. Usually I can get the thumbnail to appear by clicking on the picture next to it or by going to another folder and coming back. It's almost like a screen refresh issue. Do not know why it happens when it happens. With PortaitPro it is consistently gray until I hover on it or click the frame next to it, then it comes up.
