Lightroom 5/7 has changed behaviour in that When the Edit in "Photoshop CC 2014" in invoked, the raw file (NEF) is now opening directly in the Photoshop window as a NEF file, this is instead of a PSD or Tiff rendering being opened. Changes are made in a new file within Photoshop and cannot be returned to Lightroom by the "Save" procedure. The only way to get it back to LR is to save the new file and reimport it to Lightroom. No new copy file is generated in Lightroom. This behaviour is not replicated when the Edit in point to Elements 12, this works as it should.

If you have a matching ACR version, Lightroom sends the raw data and settings to Photoshop and opens it without first rendering a TIFF/PSD. The TIFF/PSD should be created and added back to the catalog automatically when you press Save (not Save As). This is different to the ACR mismatch or secondary editor behavior.
