Photoshop: Levels & Masks :: Distorting my PSD files, cannot "undo" to fix the issues.....

I honestly have no idea how to describe this issue. But it always happens when i've adjusted the "levels" of an image (typically when i'm editing for a texture) or when I add a layer mask. See the attached image, you'll see the side of the left has been jacked up by photoshop and then I had to recreate it on the right again.

It leaves huge square boxes all over the place, cuts off layers, adds random lines/shapes over my images etc. And the worst part is...it randomly happens and corrupts my entire file (ie: I cannot "undo" the actions that PS has took, and even if I haven't saved the file with this error it still corrupts my old saves so I can't recover my design....which leaves me having to totally redesign everything.)

Any idea when this happens? It's ridiculously frustrating and has messed up so many projects that my company has worked on.

Which version of Photoshop?
Which OS version?

That looks mostly like a video driver issue.

And if you haven't saved a file - then the original file cannot be changed and cannot have the same corruption. But a video driver problem could remain active until the app relaunches or the machine reboots.

Did you add the layer mask to a smart object with a smart filter on it?

Nope, sure didn't - They never have smart filters on them, and I always rasterize the layers.

OK, masks on Smart Filters was a known (semi-rare) bug in CS5 that was fixed in CS6.

OK, so it's not that.

And somehow whatever corruption you're seeing affects files that weren't saved - that's odd. My guess would have to be video card or bad RAM.

Hmm, that's strange. I'm a web guy - but when it comes to the inner workings of a computer i'm not so hot haha....but I have a new iMac (well, one generation old). What would you suggest doing?

Also - If this helps....I notice when I restart photoshop the issues doesn't happen. Seems like it may "cache" these weird distortions and randomly start applying them to textures....
