Photoshop: Layer confusion

I am trying to understand layers in photoshop...so I know the basics about adjustment, fill and mask layers, new and copied layers. BUT what I don't know and cant get my head around, is the basic 'rules'; why is it that sometimes I cant apply a new change onto my image. (for instance, I apply a color adjustment layer above a duplicate layer of the background, then how do I add a new layer to change the picture in another way, using the pixels?) I have tried adding a new layer and hitting COM+ALT+SHIFT+E to get the pixels up but then my change does not apply. If anyone understands what I am on about please help!!????

Why do you want to do pixel editing above the Adjustment Layer?
What kind of editing are you talking about anyway – painting, filtering, ...?

You understand that your description is worth little without at least some screenshots to illustrate what you are talking about?
What »change« exactly? Are you viewing at View > 100%?

Well i have a portrait; i want to correct the skin, hair, eyes (all on separate layers of course) and I want to change the colour and brightness etc also. So do I have to change the facial effects before I colour?

You can work however you please but if you want to work non-destructively and efficiently it would seem prudent to try and keep the layer structure as simple and straightforward as possible.

Unless you clarify what it is you are confused about (possibly with the help of some screenshots) it may be hard for anybody to help you.

ok, here is another way of explaining my confusion! (I'm sorry, bare with me, still new at this)
So say if I want to do two things to my portrait. I want to remove blemishes and I also want to sharpen areas of the face.
From the very beginning when I open my raw image, do I start by opening a new layer to do the blemishes or do I duplicate it?
When I duplicate it, make the blemishes changes, duplicate it again to make the sharpen changes, the blemishes layer will not turn on and off. Why is this? What am I missing?
I even try to make a new layer and hit COM+ALT+SHIFT+E but the same problems occurs.
