Lightroom: Importing a pictures creates a separate folder on my hard drive and copies pictures to it

Whenever I import a picture in LR, a separate folder gets created under my pictures on my hard drive and pictures are copied there. It has happened so much, my hard drive is full. I've just now figured out and can replicate what occurs and the problem seems to be with LR. I have uninstalled and reinstalled many times. Same issue. I am using version 5.7.1. I do not have auto import enabled. Backups are being saved under the backup folder and exports are being saved in a different folder than that.

First creating folders don't take up hard drive space. 
It is the images that take up the space.
Also it is the options YOU have selected in the import dialog that is causing this to happen.

The copied pictures take up the space. What options are you referring to? I can't find anywhere that I have selected this. Thanks for your help!
