When importing from camera card LR used to show only the new photo's, since some time this is broken and all photos are shown at every import, also when checking and unchecking different options. This of course is very annoying.

What version of Lightroom? Does your import session or preset have "Don't import suspected duplicates" checked?
Remember, if you import 100 images from a card, and then mark 50 of those for deletion and remove then from the catalogue, the next time you try to import from this unformatted card, it will show you those 50 deleted images plus any others added since.

It is LR 3.6 for Mac, dutch language.
It doesn't matter if "Don't import suspected duplicates" is checked or not.

Also, I am curious your workflow - are you re-importing again from card, or folder on hard drive, or what.
My understanding is that Lr uses:
Original filename + date-time original (capture timestamp) + file-size, BUT
if file imported from another catalog, original filename is (or was) not set properly.
And also file-size is particularly iffy proposition, since as soon as you re-save the file (e.g. saving xmp to DNGs or JPEGs, or saving RAW edited in mfr. software), the file-size changes.
Thus, if either of these two things happened, duplicates would not be considered duplicates.
