PHOTOSHOP - Image Adjustments do not appear to take hold. Why? How to fix?

Photoshop CS6. This is an intermittent problem but it is very serious and annoying.

Spontaneously, when I try to make an image adjustment (levels, curves, etc.) it will show the effect in the preview, but then when I cloick OK to set it, Photoshop does NOT apply the adjustment, and reverts to what it was.

I cannot say why, and it happens intermittently. It will work for a while, then it won't. It is not due to anything I cnage in my processes. It just will stop applying changes...(and then it will start doing it again for a while.

Below is a screenshot of a tree photograph showing what happens. I have exaggerated the effect deliberately for illustrative purposes.

Zoom in to 100%.
You have a very noisy image, and the adjustment preview on the downsampled image (at less than 100%) isn't going to be very accurate.

It happens on images that are less noisy too. And it often snaps back even if zoomed.

Again, zoom in to 100% to judge the effect of your adjustments.
