And that's not smoke. That's colored dye dropped into water, photographed from the side (like a fish tank, maybe with a white background) and then composited into the image with the face. It could have been any color, but was converted to black and probably set to multiply blend mode.

The face has also been colorized (one mask and color for skin, another for hair).
