How do you carve into a model when using the 3D workspace?

I'm using Creative Cloud Photoshop.  I am attempting to make a 3D model for printing and I can't seem to locate a written tutorial that has the most recent photoshop icons.  All the tutorials say CS6.  My steps so far:   I used a jpg, selected the object, extruded the layer, bezeled the top layer.  I would like to place a hole in the uppermost portion of the object but I can not find out how to make a hole.

You have to export your object and apply a hole in, blender or some other 3D program, there are some basic programs that will do just want you want with ease.
