Ever since CS4, I have had issues with the Gamut Warning. It keeps getting "greyed out". I have to start CS so that the preferences are deleted, then it works, but only for a while and then it gets greyed out again. Sure there is a work around, but can I get a fix?

I guess I am the only one with this issue?

Have you selected a profile in View/Proof Setup, at any point? If so, take a look at the Custom setup. If you have Preserve Color Numbers turned on, uncheck it and click OK. View Gamut should be available.
The other possibility is that you may have selected one of the built in profiles in the lower part of the Proof Setup menu (Legacy Mac, Internet Standard...). Those will also not allow View Gamut.

Yes, I set my Proof Set-up to the profile I am using (a paper profile). Proof Colours is on. The Gamut warning command will work for a while (not just on a particular image, but perhaps for several images) and then grey's out. The only way to get it back is to reboot CS and delete the preference file in the process.

I think part of the issue will be me. I am pretty sure that part of my issue is selecting Internet Standard for proofing on occasion, which would explain that, but it wouldn't explain the missing part when viewing a paper profile.

I don't use "preserve RGB", but because I do not, it is not something I would check for. It is quite possible that I accidentally turned it on and didn't notice, which would explain some more incidents.

I see that in the last Update, my Gamut warning is now active again. I stopped using it in my work flow because of its disappearing act. I will try it again and see if I can get more details when it happens.
