Photoshop: CS6 Pen tool & Polygonal Lasso Problem automatically changes angle of shape

When I complete a drawing and close the path then load path as selection it alters the original shape and angle sometimes splits the original shape in to three parts. See images attached. I've reinstalled PS but same thing happens. Any Ideas?

What is your View>Pixel Aspect Ratio> set to?

Hmm. That's not the problem. Is this easily reproducible? Could you record a video of all your steps to help give us more clues?

Is there an easy way to record this? It happens on all drawing tools if I draw any shape it alters as soon as the shape is complete or when I change the path to selection?

Launch QuickTime Player and select File>New Screen Recording

You can post the file to Dropbox or Creative Cloud.

What OS version are you using?
Is there any chance that you are just seeing a video card driver glitch?

No Sure about video card as if I work in CS3 I don't have the same problem, pen tool works fine?
