Create a new keyword "A", and a new keyword "A" inside of the first keyword. Go to keywording and type in "a" in the "Click here to add keywords"-box. A list with "A" and "A > A" appears, as expected.

Now rename one of the keywords you created to "a". Go to keywording and type in "a" in the "Click here to add keywords"-box. Now a list with "A" and "a" appears. Anyway which of these you choose, the first keyword you created will be added.

It appears that Lightroom is case-sensitive when displaying the list with keyword-suggestions, so "a" is different from "A" and hence the keywords that contain them are not shown ("a > A"), but when adding the keyword Lightroom is case-insensitive, so the keyword first created of "a" and "A" is added regardless of whether you type in "a" or "A".
