Can't save due to a program error

I've been having this a lot, and I've always gotten around it by merging layers until I find a combination that will save.

The PSErrorLog says "/Volumes/workarea/PS_13_Mac_Daily_Retail/20120315.r.428/photoshop/main/photoshop/xcode/../sources/UVMAccess.cpp : 778 : REQUIRE failed" but it's all greek to me.

I'm having a heck of a time trying to figure it out. CS6, OS X 10.7.3

Do the documents that won't save have anything in common (certain types of layers, perhaps)?

Chris, sorry for the delay in responding to my own problem. It had come up again once or twice, but my approach hadn't been very scientific in zeroing in on the details (more like blind panic trying to find a version that would save).

Just now I solved it in a single step for the first time, so I can say with reasonable certainty what happened.

I had a feeling that nested layer masks and adjustment layers were involved somehow, so when I couldn't save, I immediately attacked this one group (with a mask) that had only a Hue/Sat layer inside (also with a mask). The error was slightly different (can't create preview) but the result was the same from the user standpoint, I couldn't save my file.

I merged the masks, and took the adjustment layer out of the group (and discarded the group). Voila, as they say; I could save again.

The unusual and difficult to understand part is that that group was there when I opened the file in the first place. The other common factors seem to be relatively large file size (1.42 GB in this case), and possibly even CPU temperature.

If this is not reproducible, I can (and will, eventually) take another look at Jeff's suggestions.

If you can reproduce the problem again - could you take a screen shot of your layers palette and post it here? That way we can see the structure and try to reproduce it.
