Photoshop: Keyboard shortcuts reverting to older set

Any idea why my recently saved keyboard shortcut set will revert back to a previously saved set? I'll be working and all of a sudden a shortcut doesn't work and I find my new set has reverted back to another.

Using the latest CC version.

What OS are you on? Can you give me an example of a shortcut that you set and what it reverted to?

OSX 10.11.
It doesn't just revert to a previous shortcut, it reverts to a previous SET of shortcuts!

OK, this is interesting. I set up a shortcut for Select and Mask in my new set of shortcuts.

If I use that shortcut my shortcut SET reverts back to a previous SET!

Is it possible that your switching workspaces at anytime? Keyboard shortcuts are/can be saved as part of a workspace.

OK, it must be workspace related. I created a new space with the shortcut set.
I now remain in the new set when using the shortcut.

So even though shortcut was in a new set (that may or may not have been tied to the workspace, IDK) using the shortcut there reverted back to an older space even when the shortcut there had been removed? Wow.
