Lightroom: Some files give “Metadata is not up to date” but they do not have the usual badge/symbol indicating Metadata error
Some files pop up in my collection: “Metadata is not up to date”. They do not have the usual badge/symbol indicating Metadata error
I have used “Save metadata”, no luck; they pop back up.
I have tried “Save as Catalog”, delete and then Import catalog; no luck, they pop up again
I tried, export the files, delete them, then import and save metadata.

Is there anything special about these files (file type, locked status)?
You don't say whether you are using Mac or Windows. I can give some Macintosh-specific suggestions, but I know nothing about Windows.
What happens if you make a new (empty) catalog, and import just those files? Still have the problem? If so, there is probably something about the files themselves that prevents Lightroom from writing to them.
Finally, when you say "I tried, export the files, delete them, then import and save metadata." If you mean that you truly exported them, then the exported files are new files (often with reduced information) and the problem should not continue with them. But if you deleted the originals, you have likely thrown away something useful, and I would try to restore them from backups.
Sorry I can't be more specific.

Go to one of the files (click on photo, Choose menu item "Photo : Show in Finder") and then choose "Get Info" in Finder. This will show a locked check box and also some information on the bottom indicating whether you have permission to edit it.
I suspect that you have already checked this possibility, and if so, I'm out of good ideas. Perhaps you could post some screenshots and someone smarter than me could help?

This is a very old bug. Actually, it's there since version 1 of Lightroom. I have analyzed what's happening and made clear what's wrong with the timestamps in the catalog. But after all these years, we're still waiting for a fix. See here :

Anyway, once a bug in an Adobe product has lasted more than one or two years, you can bet that it will never be fixed. So you'll have to live with this one, I'm afraid, because at Adobe, nobody cares.
What you can do is the following :
- When you use Ctrl-S to save the metadata, the XMP file is correctly written (although LR pretends that it's not up-to-date, it is).
- So if you write to and then read the metadata from file, the madness will stop... for a while. But the problem will come back.
