Opening files in PS from LR, issue even with compatible LR/ACR versions
I use Lightroom 3.4.1 and Camera Raw 6.4.1, but when I choose "Open In Adobe Photoshop CS5.1" in Lightroom, Lightroom starts rasterizing a TIFF/PSD (showing the task: "Preparing file for editing") and is importing the new file to the Lightroom catalog BEFORE Photoshop is started.
I expect to at least get at dialog box with a question about what to do if the LR and ACR versions are out of sync. And since I assume 3.4.1 and 6.4.1 should be compatible, I expected the RAW file to be opened in PS/ACR and that no file is saved to disk or imported into the Lightroom catalog until I choose "Save" in Photoshop.
I installed PS+ACR just recently and before that I have been opening files from Lightroom into other external editors such as PS Elements and GIMP. I was suspecting that Lightroom were remembering settings for those old editors and I tried "Reset all warning dialogs" via Preferences -> General -> Prompts, but the behavior is still exactly the same.
I use Windows XP.

It works after I copied the file from:
C:\Program\Delade Filer\Adobe\Plug-Ins\CS5\File Formats\Camera Raw.8bi
Into:
C:\Program\Common Files\Adobe\Plug-Ins\CS5\File Formats\Camera Raw.8bi
I guess I have to remember to do this again next time ACR is upgraded.
I consider this as a workaround and think that Adobe should correct this in future versions, but now it is working for me.

Yes, you'd have to repeat this step anytime a new ACR plugin is installed. On Vista or Win7, this is no longer a problem, as the "Common Files" folder is not localized anymore.
